Comprehensive Care Conference 2023

Thank you to those who joined us at the inaugural Comprehensive Care Conference in Chicago on Thursday, October 12th. This one-day convening brought together leaders and clinicians in healthcare and social services to engage in dynamic presentations and workshops about how to address and transcend fragmentation in care with the aim of providing whole-person care and promoting wellbeing. The conference built on our mutual efforts to expand and spread comprehensive care models nationally and internationally. The convening culminated in a reflection and action steps that will inform a post-conference guide to defragment care and spread comprehensive care models to reach many more communities.
Conference Goals:
1. Convene stakeholders who have implemented or are interested in implementing models of care that defragment care for patients at increased risk of hospitalization.
2. Through conference presentations and interactive workshops, share tools and frameworks for how to successfully implement and evaluate comprehensive care models in a range of health care settings to support the dissemination of evidence-based comprehensive care models.
3. Collectively create and share deliverables for participants to continue to participate in dialogue and knowledge
